Automatic Tuition Credit Card Authorization

​Your electronic signature below may be used to enroll in monthly automatic billing instead of paying for the semester or year in full. Please note that all registered students are responsible for the entire amount of the school year in which you are enrolling regardless of attendance.  Monthly billing serves as a courtesy payment plan. Your initial payment will include your June Tuition, your Registration Fee of $25 ($40 after September 1st) and Costume Deposit of $50 (if applicable). Monthly billing for September through May will commence on September 1st.

    I have elected to pay my academic term tuition via the monthly financing plan. My initial tuition payment of two months, registration fee of $25 and a recital costume deposit of $50, will be processed upon registration. My tuition payments will be due and processed on the 1st of each month, through May of the current school year. September and June tuition payments are due September 1st.

    ​I am responsible for paying the entirety of school year tuition based on my registration form and tuition schedule as published regardless of attendance.

    My tuition is auto-renewable. Following the initial term, my membership will automatically continue on a month to month basis until my tuition balance is paid in full at the end of the school year. All cancellations and change of payment methods require 30 days written notice to Integral Ballet School, and are effective 30 days after the date received. Withdrawal policies outlined on your registration form which will be submitted with payment. Payments due prior to that date will be charged as scheduled. 

    I am responsible for alternate payment arrangements and any late or overdraft fees if Integral Ballet is unable to process my payment.

    Integral Ballet agrees to sell and I agree to purchase the membership, goods and services described in my enrollment form. I agree to pay Integral Ballet for the membership, goods and services according to the payment schedule found herein. My electronic signature below indicates my agreement to be bound by the terms, conditions, rules and regulations of this agreement. All of the terms and conditions in this agreement are a part of this agreement. All persons signing this agreement are fully responsible for paying it in full. I understand the rules and regulations and the terms in this agreement.

    I certify that I am the authorized holder and signer of the credit card referenced below. I certify that all information above is complete and accurate. I hereby authorize collection of payment for all charges as indicated above. I authorize Integral Ballet School, Inc. to charge my credit card for tuition payment or services received on the first of each month September through June, and to verify the billing address of my credit card with the issuing bank upon my signature.

    By signing this authorization, I acknowledge that I have read and am in agreement with all of the above. All information given is complete and accurate. Monthly tuition and fees will be withdrawn on or after the same day each month. I understand that Integral Ballet, Inc. may continue to charge my account or cancel my membership in accordance with the terms in this agreement. Additionally, I authorize Integral Ballet, Inc. to charge my credit card on file in lieu of presenting it for any services received, at my request.     ​
